Providence Health & Services Social Worker BSW - Long Term Care in Issaquah, Washington
Providence is calling a Social Worker (Long Term Care, Day Shift, 1.0 FTE) to Providence Marianwood in Issaquah, Washington.
We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
Schedule: Monday through Friday (8am to 4:30pm)
In this position you will:
Work collaboratively with case managers, nursing staff, external organizations, and community resources to assess the types of service, care, and treatment.
Be an advocate and provide assistance to the patient and family
Complete psychosocial patient assessments by utilizing the medical record and interviewing patients as well as their families
Link patients and families to community resources, such as Senior Services, DSHS, and other care providers
Provide counseling on such social service topics as adjusting to illness and coping with grief
Required qualifications for this position include:
Bachelor of Science degree in Social Work or Bachelor’s degree in related human services field
Current Cardiopulmonary Resuscitation (CPR) certification
2 years social service experience in geriatric and/or medical social work or a related field
Preferred qualifications for this position include:
- Master of Science degree in Social Work

Providence Marianwood in Issaquah, Washington compassionately serves people of all ages and faiths. Our Transitional Care Unit is designed for patients coming out of the hospital who need skilled nursing care, end of life care, or rehabilitation for those recovering from surgery or who have a functional loss from illness, disease, or an accident. Our Long Term Care unit provides skilled nursing in a residential setting, addressing the needs of our residents through creating a sense of community and home in “neighborhoods” and providing a model of care that is resident-centered.
